VAID RUBAL’S WELLNESS SERVICES AGREEMENT

This Agreement is entered into between Vaid Rubal’s Clinic Ltd., Brampton, Ontario (“the Clinic”) and the undersigned client (“the Client”).

By selecting a service or plan, signing, and submitting this Agreement, the Client confirms they have read, understood, and agree to the terms below.

1. SERVICES AND PLAN SELECTION

You have chosen:

Arogya Lite Plan (4 Months) Initial Fee: $590 | Monthly Fee: $90 Includes prescribed monthly Ayurvedic medicines and a 7 day diet plan.

Arogya Plus Plan (1 Year) Initial Fee: $1590 | Monthly Fee: $190 Includes prescribed monthly Ayurvedic medicines, one Ayurvedic therapy per month (Shirodhara, Nasya, or Natre Basti), a customized diet plan, and one microscope test.

Arogya Premium Plan (1 Year) Initial Fee: $2290 | Monthly Fee: $290 Includes prescribed medicines (weekly, bi weekly, or monthly as assessed), priority appointments, two Ayurvedic therapies per month (Shirodhara, Nasya, or Natre Basti), two microscope tests, a customized diet plan, custom orthotics, one full body cupping session, and one gift card.

All services and inclusions are subject to clinical assessment and suitability.
